Would you like to help our club in any of the following areas?

TERMS AND CONDITIONS

  1. By enrolling in the Satellite City United Soccer Club, you hereby provide consent for medical treatment in the event of an emergency, while understanding that every effort will be made to contact you, the parent or legal guardian, in case of such and occurrence. You also acknowledge that it is your responsibility to provide accurate and up-to-date medical information for your child, including any known allergies or medical conditions. In the absence of parental consent or in situations where immediate medical attention is required, you grant permission for club personnel to make medical decisions on your child's behalf.

  2. By submitting this form for my child, I agree to follow the rues of the Satellite City United Soccer Club and its affiliates. I understand that soccer carries some risk of injury. In exchange for my child's participation in the club's programs and activities, I release and protect the Soccer Club, its affiliates, sponsors, employees, and all related individuals. This release also covers the owners of fields and facilities used for the programs. I give consent for my child's participation and any necessary transportation. This agreement is binding and signifies my acceptance of these terms.

  3. I also grant permission for Satellite City United Soccer Club to capture photographs or videos of my child/player during club activities and events. These images may be used for promotional, educational, or informational purposes solely related to the club. I understand that Satellite City United Soccer Club and its authorized personnel have full control over the distribution and use of these images. I acknowledge that the club is not liable for any unauthorized distribution or use of these photos or videos by individuals or entities not associated with Satellite City United Soccer Club. My consent to these terms if given voluntarily and will remain in effort for the duration of y child/player's participation in the club's activities.
